POSITION OVERVIEW

The Houston Health Department (HHD) seeks a Senior Public Health Investigator in the Public Health Branch. This position requires applied expertise in client centered interviews, field investigation and other field-based activities. Which includes community outreach, collaboration with private/public medical providers, navigation of healthcare systems to ensure patient evaluation and treatment.

Job duties and responsibilities of this position include, but are not limited to, the following:

  1. Able to work in a fast-paced work environment.

  2. Will serve as a Service Linkage Worker to ensure appropriate linkage of persons newly diagnosed with HIV into primary medical care services.

  3. Maintains an active caseload of approximately 30 clients with a minimum of nine (9) new clients per month. Ensures that clients are transferred into a Medical or Clinical Case Management program within 90 days of initiation of services.

  4. Observes all confidentiality and safety regulations. Conducts confidential interviews, in both clinical and field settings, of clients diagnosed with or suspected of having a sexually transmitted disease (including HIV and Syphilis).

  5. Performs venipuncture or skin tests on an as needed basis. Conducts confidential counseling for persons who are at high risk of infection and/or transmission of HIV.

  6. Ensures all Ryan White eligibility documentation is obtained and secured in a confidential medical record.

  7. Completes case management notes in a timely manner and ensures all appropriate encounter data is entered into the Centralized Patient Care Data Management System (CPCDMS).

  8. Assigned as liaison to community-based agencies to provide technical assistance and support for STD/HIV infection activities as needed.

WORKING CONDITIONS

The position is physically comfortable; the individual has discretion about walking, standing, etc. There are no major sources of discomfort, i.e. essentially normal office environment with acceptable lighting, temperature and air conditions.

PHYSICAL SKILL: Requires the ability to make coordinated gross motor movements in response to changing external stimuli within moderately demanding tolerances; or the ability to make coordinating eye/hand movements on a patterned response space within low tolerance demands with no real speed requirements. Field activity is required. Being able to get in and out of your vehicle many times a day is necessary. Able to work in varies temperatures while in the field.

MINIMUM REQUIREMENTS

EDUCATIONAL REQUIREMENTS

Requires a Associate's degree in Social Sciences, Physical Sciences, Public Health or a closely related field.

EXPERIENCE REQUIREMENTS

Two (2) year of experience in the public health field is required.

Pertinent professional experience in the public health field may be substituted for the above educational requirement on a year-for-year basis.

LICENSE REQUIREMENTS

Requires a valid Texas driver's license and compliance with the City of Houston's policy on driving.

PREFERENCES

Preference shall be given to eligible veteran applicants provided such persons possess the qualifications necessary for competent discharge of the duties involved in the position applied for, such persons are among the most qualified candidates for the position, and all other factors in accordance with Executive Order 1-6.

• Demonstrated experience working in the HIV treatment field.

• Excellent written, oral, presentation, facilitation and interpersonal skills

• Demonstrated cultural sensitivity and ability to work comfortably with communities highly impacted by STI/ HIV, including but not limited to Lesbian, Gay, Bisexual, Transgender, and Queer (LGBTQ) individuals, gender non-conforming individuals, people who inject drugs, etc.

SALARY INFORMATION GRANT FUNDED

Grants & Special funded positions are dependent upon continued available funds. If funding is no longer available, employee may be laid off or transferred. Factors used in determining the salary offered include the candidate's qualifications as well as the pay rates of other employees in this classification.
