Key Responsibilities:

Care Coach:

• Support and mentor individuals to achieve their personal goals.

• Develop and implement personalised care plans.

• Foster a supportive and encouraging environment.

Team Leader:

• Lead and motivate a team of support professionals.

• Ensure high standards of care and compliance with policies.

• Coordinate team activities and manage daily operations.
